A Case of Unilateral Internal Ophthalmoplegia in Acute Ophthalmoplegia without Ataxia

Abstract
Acute ophthalmoplegia without ataxia (AO) is one of the anti-GQ1b antibody syndrome featured by acute postinfectious ophthalmoplegia without ataxia and elevated anti-GQ1b titers and manifests as various combinations of external and internal ophthalmoplegia. We present a 64 years-old woman with AO and unilateral internal ophthalmoplegia. She showed bilateral abduction limitation with right mydriasis. Serum antibody titers against GQ1b were elevated and no other etiology for ophthalmoplegia was found despite extensive patient evaluation. Although intravenous immunoglobulin was not administered, the patient gradually improved over subsequent months. Bilateral internal ophthalmoplegia is not uncommon feature of AO but, unilateral one is rare.
